Simplify the process to disable two-factor authentication (OD-1669)

Currrently to disable 2FA, administrator needs to disable it globaly or at group level. Then user needs to be disable it separately which is not obvious.

With the change, user can not enable/disable 2FA separately. It is only controlled at system level or group level.
